There are several methods for you to submit the forms listed below:
- Online Form (mail/fax/online): Fill in the appropriate information, review entered information and edit if neccessary, then either print from your browser and fax or mail the form with payment, or submit the form online with a credit card payment. There is an additional fee of $4.00 per dog per show for emailed or faxed entries.

We accept phone entries at 323-727-0136 until noon on closing day. There is an additional charge of $5.00 per dog per show for phone entries.

Google Chrome Users - If you are having trouble viewing the secure portion of our
site for putting in entries, try switching from Google Chrome to another web browser.
Google is working on this but has not put out any information in regards to when
this issue will be patched. Try switching to another web-browser like Safari,
Firefox, Edge, Opera, or Internet Explorer.
Dog Show Entry Form
To enter your dog(s) in an upcoming show, submit this form with the appropriate fees.
Premium List Request / Address Change Form
If you would like to be put on the mailing list to receive printed versions of the Premium List, or if you've recently moved, use this form.
Any change or cancellation must be made in writing and received prior to closing, except as provided for in Chapter 11, Section 6 of the American Kennel Club Dog Show Rules and Regulations.
Use this form to correct any information appearing in the show catalog.
Transfer and Class Change Form
Use this form for Best of Breed/Variety Transfers, 6-9 or 9-12 Class Changes, or Novice to Open Junior Showmanship Changes.
